About This Report

This report captures details around critical errors made by the original abstractor or coder, that is calculated into a Chase Score.   A critical error for HEDIS (abstractor) is defined as a change in compliance.   A critical error for Risk/IVA (coder) is defined as a missed DOS, ICD code or VRC code.   Also for Risk/IVA a critical error is defined as an added DOS, ICD code or VRC code by the original coder, that is later deleted by the over-reader.   


This report shows information by chase, original clinician, over reader, with error/correction detail. Each of these items can be filtered by measure. The errors captured are those that have been made by the original clinician and confirmed during the over read process by the over-reader.

Measure CodeDetermines the type of an associated chase, which indicates what the coder is seeking to validate
Coder User NameThe name of the user who abstracted the chart

Overread User NameThe name of the user who completed overread 1 for the given chase
Overread DateThe date that a chase was overread
Total ValidationsThe number of items evaluated by the over reader
