Create a New HEDIS Chase

TABLE OF CONTENTS

HEDIS - Existing Member, Existing Provider Address, and Existing Provider Name

  1. From Chase Query, click Create a New Chase
  2. Click Get Started
  3. Select Project
  4. Select Product
  5. Optionally, select Data Load LOB
  6. Enter a Chase Key
  7. Select Measure
    • Certain measures will require additional information to be entered
  8. Click Next Step
  9. For existing member, select Yes
  10. Use any combination of the available fields to search for a specific member
  11. Click Search
  12. From the Results table, use the radio button select your Member
  13. Click Next Step
  14. For Existing Provider Address, select Yes
  15. Use any combination of the available fields to search for a specific Provider Address
  16. Click Search
  17. From the Results table, use the radio button select your Provider Address
  18. Click Next Step
  19. For Existing Provider, select Yes
  20. Use any combination of the available fields to search for a specific Provider 
  21. Click Search
  22. From the Results table, use the radio button select your Provider 
  23. Click Next Step
  24. From here you can review the chase details, Project, Member, Member DOB, Address, Providers, and Measure. You can make changes prior to chase creation by clicking the numbers at the top.
  25. To finalize chase creation, click Finalize
  26. Click the Chase ID to access





 






 

HEDIS – New Member, New Provider Address, and New Provider 

  1. From Chase Query, click Create a New Chase
  2. Click Get Started
  3. Select Project
  4. Select Product
  5. Optionally, select Data Load LOB
  6. Enter a Chase Key
  7. Select Measure
    • Certain measures will require additional information to be entered
  8. Click Next Step
  9. For Existing Member, select No
  10. Enter required fields: Client Member ID, Member First Name, Member Last Name, Date of Birth, and Gender
  11. Optionally include: Enrollee ID, and Middle Name
  12. Click Add Member
  13. For Existing Provider Address, select No
  14. Enter required fields: Address 1, City, State, Zip Code, and Phone Number
  15. Optionally include: Address Group (PGID), Group Name, Address 2, Fax Number, Client Address ID
  16. Click Add Address
  17. For Existing Provider, select No
  18. Enter required fields: All fields are required
  19. Click Add Provider
  20. From here you can review the chase details, Project, Member, Member DOB, Address, Providers, and Measure. You can make changes prior to chase creation by clicking the numbers at the top.
  21. To finalize chase creation, click Finalize
  22. Click the Chase ID to access
